District of Columbia General Forms (NRT)-Notice of Action Taken On Claim is a form used by the District of Columbia government to notify claimants of the action taken on the claim they have submitted. The form is used to inform the claimant of the status of their claim, and includes information such as the date of action taken, the name of the payer, and the amount of the payment. There are two types of District of Columbia General Forms (NRT)-Notice of Action Taken On Claim: 1) Notice of Action Taken on Claim for Medical Assistance and 2) Notice of Action Taken on Claim for Other Assistance. The first type is used to provide claimants with information about the action taken on their claim for medical assistance, such as Medicaid, Medicare, or CHIP. The second type is used to provide claimants with information about the action taken on their claim for other types of assistance, such as food stamps, housing assistance, or Temporary Assistance for Needy Families (TANK).
