Connecticut For DSS Use Only -- Petition for Order Authorizing DSS to Enter the Premises of an Elderly Person for a Protective Services Assessment is a form that is used by the Connecticut Department of Social Services (DSS) to obtain a court order allowing them to enter the premises of an elderly person for the purpose of conducting a protective services assessment. The purpose of the assessment is to determine if the elderly person is in need of protective services or is at risk of abuse or neglect. This form is used in Connecticut and other states when the DSS cannot access the elderly person's home or other premises without a court order. There are two types of this form: one for individuals and one for couples.
