The California Information Sheet for Proof of Personal Service — Family Law is a document used to prove that a family law document has been successfully served. It is a form that is used to provide information to the court, such as the date, time, and method of service, the name and address of the person served, and the name and address of the person who served the document. The form must be signed by the server and returned to the court within 10 days of service. There are two types of California Information Sheet for Proof of Personal Service — Family Law: Regular Form FL-330 and Summons and Petition Form FL-335. The Regular Form FL-330 is used when serving documents other than a Summons and Petition, such as an Application for Order, and the Summons and Petition Form FL-335 is used when serving a Summons and Petition.
