There are two types of Arizona Petitioner's Information Sheet to Court Investigator. The first is the “Original” form, which is used to collect information from the petitioner when they first submit their case to the court. The second is the “Supplemental” form, which is used to collect additional information from the petitioner when their case is referred to an investigator.
