Arizona Affidavit of Person to Be Appointed Guardian and/or Conservator is a legal document used in the state of Arizona to appoint a person to serve as a guardian and/or conservator of an incapacitated individual. This document is typically used when an individual is unable to manage his or her own affairs due to mental or physical incapacity. It is also used to appoint a guardian and/or conservator of a minor or adult who is unable to manage his or her own affairs. There are two types of Arizona Affidavit of Person to be Appointed Guardian and/or Conservator: (1) Guardian of a Minor and (2) Guardian and/or Conservator of an Adult. The Guardian of a Minor Affidavit is used to appoint a legal guardian for a minor who cannot manage his or her own affairs due to age. The Guardian and/or Conservator of an Adult Affidavit is used to appoint a legal guardian and/or conservator for an adult who cannot manage his or her own affairs due to mental or physical incapacity. The affidavit must be signed and witnessed by two individuals and must include the name and address of the proposed guardian and/or conservator, the name and address of the incapacitated individual, a statement of the reasons why the appointment is in the best interests of the incapacitated individual, and a statement that the proposed guardian and/or conservator is qualified and suitable to act in that capacity.
