The Alaska Screening Investigation Billing Form is a document used to collect billing information from businesses and individuals who have requested certain screening investigations from the State of Alaska. The form is used to provide billing information for investigations requested by public and private entities, including but not limited to background checks, criminal history checks, and drug testing. The form contains fields for the requestor’s name, address, and contact information; the type of investigation requested; the purpose of the investigation; the cost of the investigation; the billing address; and any additional information or comments. There are three types of Alaska Screening Investigation Billing Forms: the Individual Form, the Business Form, and the Drug Test Request Form. The Individual Form is used for individual requests for background checks, criminal history checks, and drug testing, the Business Form is used for requests from businesses and organizations, and the Drug Test Request Form is used to request drug testing.
