Wyoming MCR 4. Parental Consent to Appointment of Guardian is a form that is used in the state of Wyoming in order to obtain parental consent for the appointment of a guardian. This document is used to establish a guardian-ward relationship in cases where the parent(s) of a minor or incapacitated individual are unable to care for them. This form must be signed by the parent(s) and notarized in order for the court to consider the appointment of a guardian. There are two types of Wyoming MCR 4. Parental Consent to Appointment of Guardian: 1) MCR 4A — Consent to Appointment of Guardian for Minor; and 2) MCR 4B — Consent to Appointment of Guardian for Incapacitated Person.
