Wyoming CAMP 13. Application for Entry of Default is a form used by creditors to enter a default judgment when a debtor fails to respond to a lawsuit or debt collection action. The form must be signed by the creditor and notarized. There are two different types of Wyoming CAMP 13. Application for Entry of Default: 1) Uncontested, where the debtor does not file an answer or other response to the lawsuit; and 2) Contested, where the debtor has filed an answer or other response to the lawsuit. The form must be completed with accurate information and filed with the court where the lawsuit is pending.
