The West Virginia Controlled Substances Form (CFS) is a document used by pharmacies, hospitals, medical providers, and other healthcare professionals to track the dispensing of controlled substances within the state. The CFS is used to document the dispensing and receipt of controlled substances, including opioids, stimulants, and depressants, among other substances. There are two types of CFS: the Original Controlled Substance Form and the Replacement Controlled Substance Form. The Original Controlled Substance Form is used for the initial receipt of the controlled substances, and the Replacement Controlled Substance Form is used when a controlled substance is dispensed and replaces a previously dispensed controlled substance. The CFS must be completed and submitted to the West Virginia Board of Pharmacy within three days of the date of dispensing.
