The Wisconsin Foreign Business Corporation — Withdrawal and Final Report Application is a form used to withdraw a foreign business corporation from the state of Wisconsin. This application is used by foreign corporations to dissolve their registration in Wisconsin and to provide a final report to the state. The application must be filed with the Wisconsin Department of Financial Institutions.
