The Washington Cover Sheet — Family Cases is a form used by attorneys filing cases in family law courts in the state of Washington. It is designed to provide the court with essential information about the case such as parties involved, case type, relief sought, and attorneys of record. It also helps the court to decide if the case requires additional filing fees or special procedures. There are two types of Washington Cover Sheet — Family Cases: Dissolution and Legal Separation. The Dissolution form is used for filing actions such as divorce, annulment, and dissolution of domestic partnerships. The Legal Separation form is used for filing actions such as legal separation, separate maintenance, and non-parent custody.
