The Utah Designation of Record On Appeal (Appeal To District Court) is a form that is used by a party to appeal a case from a lower court to a district court. It is also known as the Notice of Appeal. This form must be filed with the clerk of the lower court within ten days of the lower court's decision. It must include the names of the parties involved, a detailed statement of the facts in the case, and a list of the documents that are being appealed.
