The Utah Reinstatement Application is a document used by individuals who have had their license suspended or revoked in Utah to apply for reinstatement. Depending on the individual's driving history, this application may be used to apply for either a full or limited reinstatement of their license. The Utah Reinstatement Application consists of three parts: the Driver License Reinstatement Application Form, the Financial Responsibility Statement, and the Signature and Release.
