South Carolina Request to Correct Data Entry Error is a form used by individuals to request corrections to any incorrect data entered the state's records, such as birth certificates, death certificates, marriage certificates, and other vital records. The form must be completed by the individual requesting the correction and must include the original record document, a written explanation of the correction needed, and any additional documentation that supports the correction request. The form can be obtained from the South Carolina Department of Health and Environmental Control (DEC). There are two types of South Carolina Request to Correct Data Entry Error: (1) Request to Amend Birth Certificate and (2) Request to Amend Death Certificate.
