The Pennsylvania Electronic Data Interchange (EDI) First Report of Injury (FROM) is a secure, electronic system used by employers, insurers, and other entities to report workplace injuries or illnesses to the Pennsylvania Department of Labor & Industry (DLI). The EDI FROM is a standardized, electronic format that allows data to be easily exchanged among the various parties involved in the claim process. The EDI FROM is divided into two different types: Type 1 and Type 2. Type 1 FROM is used to report any incident that results in a fatality, a hospitalization that lasts more than 24 hours, an amputation, or the loss of an eye. Type 2 FROM is used to report any incident that results in an inpatient hospitalization of less than 24 hours, medical treatment beyond first aid, or a lost work day due to injury or illness. The EDI FROM provides employers, insurers, and other entities with an efficient way to report workplace injuries and illnesses to the Pennsylvania DLI. The EDI FROM also helps the DLI to accurately track and analyze workplace injuries and illnesses in order to identify areas of improvement in safety and health standards in the workplace.
