Pennsylvania Death Correction Statement is a form used to correct errors in death certificates issued by the Pennsylvania Department of Health. This form is used to make corrections to the decedent's name, date of birth, age, place of death, date of death, cause of death, or other information on the death certificate. There are two types of Pennsylvania Death Correction Statement: Amendment and Correction. The Amendment form is used when an error is found on the death certificate and requires a signature from the informant. The Correction form is used when an error is found on the death certificate and does not require a signature from the informant. Both forms must include supporting documentation to prove the change is accurate.
