The Pennsylvania Statement of Revival [DSC:15-1341/5341] is a document which is used by businesses in Pennsylvania to reactivate their charter or corporate status. It can be used to revive a business which has been dissolved due to not filing annual reports or failing to pay taxes to the state. The statement must be signed by all registered officers of the business and must be filed with the Pennsylvania Department of State, Bureau of Corporations and Charitable Organizations. There are two types of Pennsylvania Statement of Revival documents: the Statement of Revival of Corporate Status and the Statement of Revival of Charitable Status.
