The Oregon Waiver of Personal Service (6D) is a document used in the state of Oregon in order to waive the service of process for certain civil matters. It is most commonly used to indicate that a defendant has been provided with a copy of the relevant documents and has agreed to waive the requirement that they be served with those documents in person. This document must be completed, signed, and filed with the court in order to be valid. There are two types of Oregon Waiver of Personal Service (6D): 6D(1) and 6D(2). 6D(1) is used when an attorney is representing the defendant, while 6D(2) is used when the defendant is not represented by an attorney. Both types require the defendant's signature and the signature of their attorney, if applicable.
