The Oregon Confidential Information Form (CIF) for Respondents is a form used to protect the confidential information of individuals or organizations participating in legal proceedings. There are three types of Oregon Confidential Information Forms (CIF's) for Respondent: the initial CIF, the supplemental CIF, and the amended CIF. The initial CIF is used to request confidential information from the respondent and must be completed and signed by the respondent before any other documents can be made available. The supplemental CIF is used to update or supplement the initial CIF if new information is needed. The amended CIF is used to update or modify the initial CIF if changes are necessary.
