The Oregon Confidential Information Form (CIF) for Petitioner is a document that must be completed by a person petitioning a court for any relief. It is used to protect the petitioner’s confidential information, such as address and phone numbers, from being made public in the court proceedings. The form must be signed by both the petitioner and the attorney and filed with the court prior to any hearing or other court proceeding. There are three different types of Oregon Confidential Information Forms (CIF's) for Petitioner: the Notice of Filing CIF, the Motion for Protective Order, and the Order for Protective Order. The Notice of Filing CIF is filed with the court in order to inform the court of the petitioner’s desire to keep certain information confidential. The Motion for Protective Order is a request that the court issue an order to protect the confidential information from being made public. Finally, the Order for Protective Order is the court’s ruling on the petition.
