The Oklahoma Notice of Party Name Correction (CV-26) is a document filed with the court by parties to a lawsuit who wish to change their name as it appears in the pleadings. There are two types of Oklahoma Notice of Party Name Correction (CV-26): a (CV-26A) form for individuals and a (CV-26B) form for corporations, partnerships, and other unincorporated associations. The CV-26A and CV-26B forms must be signed by the party or an attorney representing them, and filed with the court at least 10 days prior to the scheduled hearing date. The Notice of Party Name Correction must include the name of the party as it appears in the pleadings, the proposed new name, and the reason for the change. It must also be accompanied by a copy of the identification card, such as a driver's license or state identification card, of the individual whose name is being changed.
