The Oklahoma Certificate of Service (CV-10) is a document issued by the Oklahoma Department of Public Safety that provides proof that a driver has completed the required number of hours of driving instruction and has passed the state's vision, written and driving tests. The CV-10 is required to obtain a learner's permit, a driver's license, or an identification card in the state of Oklahoma. There are two types of Oklahoma Certificate of Service (CV-10): the CV-10A, which is issued to individuals who are 18 years of age or older, and the CV-10B, which is issued to individuals who are under the age of 18. Both forms must be completed in full and submitted to the Oklahoma Department of Public Safety in order to receive a valid Oklahoma driver's license.
