The New York 5 Question Sheet is a document created by the New York State Education Department to help teachers assess student learning in the five core subject areas (English Language Arts, Math, Social Studies, Science, and the Arts). It is designed to be used as a post-assessment tool to measure student understanding of the material they have been taught. The sheet is divided into five sections, each corresponding to one of the five core subject areas. Each section contains five questions that are written in a multiple-choice format. Depending on the grade level, the questions may range from basic recall to more complex critical thinking questions. There are three versions of the New York 5 Question Sheet: one for Grades 3-5, one for Grades 6-8, and one for Grades 9-12.
