The New York State Case Registry Filing Form (SCOFF) is a form used by attorneys to file cases in the New York State courts. This filing form allows attorneys to register a case with the New York State Unified Court System (UCS) and request a case number for the case. The form has two parts, a cover form and an information form. The cover form contains basic information about the case, including the names of parties involved, the court where the case is being filed, and the case type. The information form contains more detailed information about the case, such as the facts of the case and the parties’ positions. There are four types of New York State Case Registry Filing Forms: Civil, Family, Criminal, and Other.
