The New York CD-11/CD-11 Price Increase Form is a document used to request a price increase for goods or services provided by a vendor in the state of New York. This form is required for any increase in the price of goods or services that are to be sold to the state and must be submitted by the vendor at least 30 days prior to the proposed increase. There are two types of New York CD-11/CD-11 Price Increase Forms: the CD-11A for price increases of 5% or less and the CD-11B for price increases of more than 5%. The form must include the vendor’s name, address, and contact information; the proposed price increase; the reasoning for the increase; and supporting documentation. The form must be signed and dated by an authorized representative of the vendor before it can be submitted for approval.
