The New York State SD-1 Form is a document used by employers to report new hire and rehire information to the New York State Department of Labor, as required by the Child Support Standards Act. It is used to help establish, modify, or enforce child support orders, as well as to locate non-custodial parents. There are three different types of New York SD-1 Forms: the New Hire Form, the Rehire Form, and the Termination Form. The New Hire Form is used to report information about a new employee, such as their name, address, Social Security number, date of hire, and wages. The Rehire Form is used to report the re-employment of an employee previously reported using a New Hire Form. The Termination Form is used to report the termination of an employee.
