The New York Form ADV-W Notice of Withdrawal from Registration as an Investment Advisor is a form used by an investment advisor registered in the State of New York to withdraw its registration. It must be completed and filed with the State of New York Department of Financial Services. The form is intended to inform the Department that the investment advisor no longer wishes to be registered and thus no longer requires the Department’s oversight. There are two types of New York Form ADV-W Notice of Withdrawal from Registration as an Investment Advisor. The first type is the Full Withdrawal Notice, which is used when the investment advisor no longer wishes to be registered in the State of New York. The second type is the Partial Withdrawal Notice, which is used when the investment advisor wishes to remain registered in the State of New York but is only withdrawing registration for certain services or products.
