The New York Affidavit of Financial Disclosure — Parents (Private-Placement) is a document that is required by the New York State Department of Family Services in order for a private-placement adoption to take place. This affidavit is intended to provide an accurate and complete financial disclosure by the prospective adoptive parents and/or their agents. The affidavit must be completed and notarized, and must include detailed financial information such as income, assets, liabilities, and expenses. The affidavit must also include a list of any and all financial assistance received by the adoptive family. There are two types of New York Affidavit of Financial Disclosure — Parents (Private-Placement): the first is for an adoption of an infant, and the second is for an adoption of an older child.
