A New York Notice of Appeal (Workers Compensation) is a document used to formally challenge a decision made by the Workers' Compensation Board in the state of New York. This document serves to notify the Board of the appellant's intent to appeal a prior decision and outlines the basis of the appeal. There are two types of New York Notice of Appeal (Workers Compensation): the Notice of Appeal from a Workers' Compensation Board Decision and the Notice of Appeal from a Workers' Compensation Law Judge Decision. In both cases, the Notice must be in writing and must include the appellant's name and address, the date of the decision being appealed, and the reason for the appeal. The Notice must also include a statement of the facts which support the appellant's grounds for appeal, and must be signed by the appellant or their attorney.
