New Jersey Rule 5:4-2(h) Certification by Attorney and Client — Appendix XXVII-A is a form used by attorneys and their clients in New Jersey to certify the accuracy of the documents submitted in a civil lawsuit or motion. This certification must be signed by both the attorney and the client and must include a statement that the documents submitted are true, correct, and complete to the best of the signatory's knowledge. There are two types of New Jersey Rule 5:4-2(h) Certification by Attorney and Client — Appendix XXVII-A: one for individual litigants and one for corporate litigants. The form must include the name of the court, the case number, the parties involved, the type of document being certified, and a statement that the document was prepared in accordance with the New Jersey Rules of Civil Procedure.
