The Pennsylvania Tax Lien Certificate Request Form (55.4 KiB) is a form used by taxpayers to request a Certificate of Tax Lien. The Certificate of Tax Lien is issued by the Department of Revenue to certify that a tax lien has been imposed on a taxpayer's property. Taxpayers can use this form to request a Certificate of Tax Lien as a way to prove that taxes are paid and that they have no outstanding liabilities. The form also allows taxpayers to request a Certificate of Release of Tax Lien if they have already paid their taxes and want to remove the lien from their property. There are two types of Pennsylvania Tax Lien Certificate Request Forms — one for individuals and one for businesses. The form includes sections for the taxpayer's name, address, Social Security number, and other required information. Once the form is completed, it must be submitted to the Department of Revenue.
