New Jersey 10889 — Rule 5:4-2(h) Certification by Self-Represented Litigant — Appendix XXVII-B is a form to be completed and signed by an individual who is representing themselves in a legal matter. This form is used to certify that the individual is acting as their own legal representative and has read and understood the applicable rules of court in the matter. The form must be signed and notarized before it can be accepted by the court. There are two types of this Certification: Form A and Form B. Form A is used when the individual is representing themselves in a civil manner. Form B is used when the individual is representing themselves in a criminal matter.
