The New Jersey Trial De Novo Request Form is a legal document used to appeal a municipal court judgement to a higher court. This form must be completed and filed in order to appeal a lower court ruling and request a “triaNovatovo”, or a new trial, before a higher court. There are two different types of New Jersey Trial De Novo Request Forms. The first type is the “Local” form, which is used to request a trial de Nova in a county or district court. The second type is the “State” form, which is used to request a trial de Nova in the New Jersey Superior Court. Both forms require the appellant’s name and address, as well as information about the lower court’s ruling and the relief requested. The forms must be filled out completely and filed with the proper court in order for the appeal to be valid.
