The Nebraska Temporary Child Information Affidavit (CIA) is a state-specific document used to provide temporary guardians with information about a child they are caring for. This document is typically used when temporary guardians are caring for a child for an extended period of time, such as when a parent is out of town or unable to care for the child for some reason. The CIA is used to provide information to the temporary guardian regarding the child’s medical history, educational needs, religious preferences, contact information for parent(s), and any other relevant information. There are two types of Nebraska TCAS: the General CIA and the Special CIA. The General CIA is used when the temporary guardian is a friend or relative of the child’s parent(s), while the SpeciaCIAIA is used when the temporary guardian is not related to the parent(s). Both forms are available from the Nebraska Department of Health and Human Services, and they must be completed prior to the temporary guardians taking custody of the child.
