North Dakota Absentee/Mail Ballot Application-Agent Authorization is a form that allows a voter to request an absentee or mail ballot. The form is used to appoint an agent who can obtain the ballot from the county auditor and deliver the ballot in a sealed envelope to the voter. The form must be notarized and returned to the county auditor. There are two types of North Dakota Absentee/Mail Ballot Application-Agent Authorizations: one for voters who are registered or preregistered in North Dakota and one for voters who are not registered or preregistered in North Dakota. The form for voters who are not registered or preregistered in North Dakota also requires a copy of valid identification.
