Montana Mont. LBF 38-A. AFFIDAVIT IN SUPPORT of MOTION FOR ENTRY of DISCHARGE (CHAPTER 12) [MONT. LB 4004-1] is an affidavit form used in the state of Montana to support a motion to obtain a discharge under Chapter 12 of the Bankruptcy Code. This form is used by debtors to certify that they have met all the requirements of the Bankruptcy Code, and they can be discharged from their debts. The form contains several sections which require a debtor to provide information including their name, address, Social Security number, the date of filing, a statement of their financial condition, and a list of creditors and the amount owed to each.
