Montana Mont. LBF 26. REQUEST FOR IN-PERSON HEARING [MONT. LB 5074-1(a)] is a form used to request an in-person hearing before a department of labor official. This form must be submitted by a person who is either an employer or employee and is requesting a hearing on a dispute involving wages, hours, overtime, or other labor issues. This form can be used for all types of labor disputes, including those involving individual employees, collective bargaining agreements, and employee classifications.
