Minnesota Receipt of Minor Settlement Order and Funds (Form 145.1) is a form issued by a court in the state of Minnesota after a minor has received a settlement from a legal dispute. The form is used to document the receipt of the settlement funds by a minor and is signed by the court-appointed guardian of the minor, or conservator. This form is also known as a Minnesota Order on Minor’s Compromise and Settlement or a Minnesota Minor’s Settlement Order and Receipt. There are two types of Minnesota Receipt of Minor Settlement Order and Funds (Form 145.1): 1) The “Court Approved” version of the form is used when the court has approved the settlement agreement and the funds have been received. 2) The “Court Under Review” version of the form is used when the court is still considering the settlement agreement and the funds have not yet been received.
