Minnesota Digital Audio Request Form is a document used to request digital audio recordings from the Minnesota State Archives. It enables patrons to select a specific audio recording to borrow, obtain copies of digital audio recordings, and access audio recordings stored in the Minnesota State Archives' Digital Audio Repository. There are two types of Minnesota Digital Audio Request Form: a Borrow Request Form and a Copy Request Form. The Borrow Request Form is used to request a digital audio recording to be borrowed from the archives. It requires a patron to provide their name, address, telephone number, email address, and the title of the audio recording they wish to borrow. The Copy Request Form is used to request copies of digital audio recordings from the archives. It requires a patron to provide the same information as the borrow request form as well as the type of format they would like the audio recordings to be provided in. Both forms require the signature of the patron in order to be processed.
