New York Verification (Form 4-13) is a form used by employers in the state of New York to verify the identity and work eligibility of newly hired or rehired employees. The form must be filled out and signed by the employee and employer, and then returned to the New York State Department of Taxation and Finance. The form is used to verify that the employee is legally authorized to work in the United States. There are two types of New York Verification (Form 4-13): the New York Employment Verification (Form 4-13a) and the New York Wage and Tax Statement (Form 4-13b).
