The Maine Reaffirmation Agreement Cover Sheet (B427)(12/15) is a form that is used by debtors filing for bankruptcy in the state of Maine. It is used to provide the court with the necessary information about the debtor’s debts that they are reaffirming and the creditors that are agreeing to the reaffirmation. The form must be signed by both the debtor and the creditor in order for it to be valid. There are two types of Maine Reaffirmation Agreement Cover Sheet (B427)(12/15): the Joint Cover Sheet and the Individual Cover Sheet. The Joint Cover Sheet is used when the debtor and the creditor are jointly filing for bankruptcy, while the Individual Cover Sheet is used when the debtor is filing for bankruptcy alone. Both forms are required to be filled out in full and signed by both parties in order for the reaffirmation agreement to be valid and accepted by the court.
