A Kentucky Transcript Order Form is a document used for requesting and obtaining a transcript from the Kentucky Department of Education. It consists of two main parts: the Requestor Information section, which requires the student’s name, address, and other contact details; and the Transcript Information section, which requires the requested payment amount, the school from which the transcript is requested, and the purpose for the transcript. There are two types of Kentucky Transcript Order Form: an online form for ordering a transcript electronically, and a paper form for ordering a transcript by mail. Both forms require the submission of a signed release authorizing the Kentucky Department of Education to release the transcript.
