Indiana Identification Form- Individual is an identification form issued by the Indiana State Department of Motor Vehicles (BMV). It is used to establish identity for driver’s licenses and state identification cards. The form is available in two versions: a standard version for non-commercial driver's license and identification cards, and a commercial version for commercial driver's license applications. Both versions require applicants to provide basic personal information, such as name, address, date of birth, and Social Security number. For the standard version, applicants must also submit a photograph and proof of identity, such as a valid driver’s license or passport. There is no fee for the form.
