The Illinois Class 9 Affidavit (2015) is a document used to transfer a title of a motor vehicle from one person to another. It is filled out by the seller of the vehicle and submitted to the Secretary of State along with the other required paperwork. The affidavit is an official document that must be notarized and includes the name of the seller, the vehicle's make, model, year, and VIN, the odometer reading, and whether the vehicle is being sold "as is" or with a warranty. The affidavit also serves to identify any liens that may be held on the vehicle. There are two types of Illinois Class 9 Affidavit (2015): the Standard Affidavit of Vehicle Sale and the Affidavit of Vehicle Donation.
