Hawaii Continuation Sheet is a form used by the Hawaii Department of Taxation to report additional information related to a taxpayer's Hawaii income tax return. It is used to report additional income, deductions, and/or credits that cannot be reported on the original tax return. There are two types of Hawaii Continuation Sheets: Form N-11 and Form N-13. Form N-11 is used to report additional income and deductions, while Form N-13 is used to report additional credits. The Hawaii Continuation Sheet must be completed in its entirety and signed by the taxpayer to be accepted by the Department of Taxation.
