The Alaska Plea Form (For Mandatory Appearance Offenses Only) is a legal document that is used when a defendant is charged with a mandatory appearance offense in the state of Alaska. This form is used to enter a plea of guilty, no contest, or not guilty to a criminal charge, and to waive the right to a jury trial. There are two types of Alaska Plea Form (For Mandatory Appearance Offenses Only): the "Plea Form" and the "Plea Form with Waiver of Jury Trials." The Plea Form is used to enter a plea without waiving the right to a jury trial, while the Plea Form with Waiver of Jury Trials allows a defendant to enter a plea and waive the right to a jury trial at the same time. Both forms must be signed in the presence of a court clerk and/or a judge.
