Alaska Plea Form (For Optional Appearance Offenses Only) is a document used by individuals charged with an optional appearance offense in the state of Alaska. This form is used to enter pleas of guilty, not guilty, or no contest. It is also used to request a continuance, waive a jury trial, or to request a suspended imposition of sentence. This form is only applicable to optional appearance offenses, which are misdemeanors and non-felony violations which do not require the defendant’s physical presence in court. This form is available in two versions: one for individuals who are represented by an attorney, and one for individuals who are not represented by an attorney. Both forms require the defendant to enter a plea of guilty, not guilty, or no contest to the charges. On the form, the defendant must also provide their name, address, birthdate, and other relevant information. In addition, the defendant must indicate whether they would like to waive a jury trial or request a suspended imposition of sentence. Finally, the defendant must sign the form in the presence of a court clerk or notary. After the form is complete, the defendant must submit it to the court.
