Description

This form is a Renunciation and Disclaimer of the proceeds of a Life Insurance Policy or an Annuity Contract. Upon the death of the decedent, the beneficiary gained an interest in the proceeds of the policy or the contract. However, pursuant to the Mississippi Code, Title 89, Chapter 021, the beneficiary has chosen to disclaim his/her interest in the proceeds. Therefore, the proceeds will devolve to others as though the beneficiary predeceased the decedent. The form also contains a state specific acknowledgment and a certificate to verify delivery.
