Please do not use correction fluid or similar products on a passport application form as it will invalidate that section of the form and you will have to complete another form. Instead, just cross out the incorrect information and sign next to it. Then correctly complete the section.
To correct a data or printing error, submit the following by mail: Form DS-5504 (find mailing instructions on the form) Your current passport. One color photo. Evidence of the error (e.g. your U.S. birth certificate showing the correct spelling of your last name). You will not have to submit any fees.
Entry of applicants into the Passport Office is not prohibited to make any correction in their documents or to get any updates. If any correction is needed after submission of the application, applicant can visit enquiry counter in the RPO concerned either by scheduling an appointment online or walk-in.
You may renew your current passport on DS-5504 for any of the following special circumstances: Your current passport is limited and was issued in an emergency less than one year ago, and the letter issued at that time specifically mentions you can apply using a DS-5504 application form.
