Copies of any documents filed with the Department of State's Division of Corporations may be obtained by submitting a written request to the New York State Department of State, Division of Corporations, One Commerce Plaza, 99 Washington Avenue, Albany, NY 12231.
Exemplification is required for apostille and authentication. Contrary to what many apostille agencies state on their websites, without the Letter of Exemplification NYC Birth Certificates and Death Certificate cannot be apostilled or authenticated.
A certification from a judge of the superior court which attests that their signature is authentic, and can be trusted. Only an authorized personnel can attest or execute the exemplification.
If you need a Letter of Exemplification to get an Apostille, you should order a long form birth certificate. When you place your order for the birth certificate, make sure you clearly explain the reason for your request, and the Department of Health will determine if a Letter of Exemplification is also needed.
